Letter of Intent/Application Process
The Live Well Foundation invites South Lake County nonprofits to apply for a grant to help achieve our goal of providing much-needed health and wellness programs for South Lake County residents. Printable RFP’s are attached and available below by clicking on the desired grant cycle.
How To Apply
Register your organization using our Grant Portal to complete the letter of intent.
- A letter of intent (LOI) is mandatory to be considered for the full application.
- Complete all parts of the LOI by the deadline.
- Late or incomplete LOI cannot be considered for funding.
- If LOI is accepted, complete the full application by the deadline.
- Late or incomplete applications cannot be considered for funding.

Learn About Our Grant Cycles
January 2026 – Special Populations Grant Cycle:
- Individuals Facing Barriers to Access: People who experience physical, cognitive, language, transportation, or financial barriers that limit access to services and supports.
- People with Disabilities or Chronic Conditions: Individuals with physical, developmental, intellectual, cognitive, or behavioral health conditions requiring specialized services or accommodations.
- Individuals Experiencing Life Transitions or Crisis: People impacted by homelessness, family instability, disaster, violence, incarceration, serious illness, domestic violence, or sexual abuse.
- Caregivers and Support Networks: Unpaid caregivers, family members, and support persons who play a critical role in the well-being of others.
March 2026 – Nutrition & Healthy Eating Grant Cycle:
- Access to Nutritious Food: Ensuring individuals and families have consistent access to affordable, culturally appropriate, and nutrient-dense foods.
- Nutrition Education & Skill Building: Programs that build knowledge around healthy eating, meal planning, cooking, and food literacy.
- Food Security & Stability: Initiatives addressing hunger and food insecurity through meals, food distribution, and community feeding programs.
- Equitable & Sustainable Food Systems: Community-driven efforts addressing food deserts, transportation challenges, and inequitable food environments.
June 2026 – Mortality & Life Expectancy Grant Cycle:
- Preventable Death Reduction: Programs focused on early detection, access to care, and timely intervention for preventable causes of death.
- Chronic Disease Management & Prevention: Efforts addressing heart disease, cancer, diabetes, respiratory illness, stroke, and Alzheimer’s.
- Access to Timely & Quality Care: Initiatives improving access to primary care, specialty services, emergency response, and continuity of care.
- Health Equity & Disparity Reduction: Programs focused on closing life expectancy gaps related to race, income, geography, disability, and social determinants of health.
September 2026 – Mental Health Grant Cycle (All Ages):
- Youth & Adolescent Mental Health: Programs addressing anxiety, depression, trauma, behavioral health, and social-emotional development.
- Access to Care: Expanding availability of counseling, therapy, psychiatric services, and tele-mental health.
- Crisis Intervention & Stabilization: Suicide prevention, crisis hotlines, mobile crisis units, and post-crisis follow-up services.
- Trauma-Informed Care: Programs supporting individuals affected by ACEs, violence, abuse, displacement, or chronic stress.
