Easterseals Florida Expands Disability Health Navigator Program with $250,000 Grant from Live Well Foundation of South Lake

Two-year grant funds bilingual care coordination and healthcare access services for people with disabilities across South Lake County

The Live Well Foundation of South Lake has awarded a $250,000 grant over two years to Easterseals Florida Inc. to expand its Disability Health Navigator Program, increasing access to personalized healthcare services for residents with disabilities in South Lake County, the foundation announced April 15.

The program, led by President and CEO Bev Johnson, will serve clients of all ages through a team of four bilingual health navigators who provide care coordination and advocacy in community-based settings. Services are tailored to the individual, meeting clients where they are geographically and based on their specific healthcare needs.

The Disability Health Navigator Program addresses gaps in healthcare access by helping individuals obtain insurance coverage, screening children for developmental delays, providing immunization education, and reducing systemic barriers that delay or prevent care. The initiative is designed as a scalable model capable of serving a growing and diverse regional population.

The expansion is supported in part by a pending grant from the Orlando Magic Youth Foundation.
