At Community Health Centers, integrated care means treating the whole person—and sometimes, that begins with simply listening.
During a routine visit, a patient shared she was still struggling with grief and depression. She was hesitant to talk further, fearing her symptoms might worsen—and carrying the weight of a negative counseling experience in her past.
Thanks to Community Health Centers’ on-site Licensed Clinical Social Worker (LCSW), funded in part by a grant from the Live Well Foundation of South Lake, help was just a hallway away.
The LCSW stepped in, offering 15–20 minutes of compassionate listening, support, and encouragement. By the end of that visit, the patient—still hesitant—agreed to try counseling again.
Today, she’s thriving.
She now attends bi-weekly sessions, has built a strong relationship with her LCSW, and is making real progress: navigating her grief, repairing her marriage, expressing emotions safely, and embracing her authentic self.
She shared that without that initial connection, she might never have returned to therapy.
This is the power of integrated care—and the impact of trusted community partnerships.
We’re proud to support Community Health Centers as they continue to expand access to whole-person care across South Lake.
