Here’s a story that reminds us what community care really looks like.

This past September, Easterseals’ incredible Health Navigators helped bridge the gap between Hands of Hope America and the Lake County Health Department, making sure that a vital service—free flu vaccines—was available during the Community Outreach Event on September 25, 2024.

And it couldn’t have come at a better time. In a county where many still go without vaccinations, this partnership made a meaningful difference by protecting more residents from preventable illness. That’s the kind of behind-the-scenes magic we don’t always see—but it’s changing lives.

But Easterseals didn’t stop there. Their Health Navigators have been busy forging strong connections with fellow nonprofits, including some of our own beloved Live Well Foundation grantees. These partnerships mean more shared resources, better access to health information, and a stronger support network for the South Lake community.

Throughout 2024, they’ve sent out hundreds of timely messages—from vaccine reminders and holiday blood sugar tips to the importance of staying on top of heart health. Every note, every resource shared, is a reminder that someone is looking out for our neighbors’ well-being.
